Dr. Chen is currently accepting new patients. If you are interested in making an appointment, we invite you to take advantage of our convenient Online Appointment Request Feature. Appointments are available on a first-come first-served basis. It highly recommended that you schedule your next periodic 3, 4, or 6 month cleaning appointment as early as possible; especially if you have limited availability. Most of our patients schedule their appointments 3 to 6 months in advance. Dr. Chen's office hours are by appointment only. We know your time is valuable, so we prefer to schedule longer appointments and complete the treatment you want in as few visits as possible.
We accept VISA, MasterCard, American Express and Discover cards, as well as CareCredit financing (Including CareCredit 0%* deferred interest plans)
Patients without Dental Insurance
We can provide you a cost estimate over the phone for a new patient exam and x-rays only. Any other costs cannot be estimated before you've had your appointment with Dr. Chen for an exam and necessary x-rays. Payment in-full is due at the time services are rendered. We do not accept partial payments.
Patients with Dental Insurance
As a courtesy to you, our office will gladly submit claims to your insurance. However, please keep in mind that all services rendered whether or not covered by insurance, are ultimately the financial responsibility of the account holder. We are able to submit claims to most indemnity insurance plans. We do not accept DMO or DPO plans (Dental Maintenance or Dental Provider Organizations). Under these plans, there is no coverage when treatment is rendered by a non-participating dentist. Please check your type of plan carefully. For more specific information about in or out-of-network benefit amounts, please call your insurance company.
Please let us know in advance if you'd like us to contact your insurance to request an “estimate” of your patient portion either verbally or by electronically submitting a pre-estimate of services. We cannot guarantee the payment of insurance benefits nor can we provide 100% accuracy of any estimated insurance benefits. Payment of your “estimated” portion is due at the time services are rendered, such as your annual deductible and/or co-pay. We will give your insurance company 60 days to remit payment. If there is still no payment after this time then a statement will be sent to you, and payment in full will be due on the date printed on the statement. It is the responsibility of the account holder to contact their own insurance company regarding the non-payment of a claim. Should our office eventually receive a payment from the insurance after it has been paid by you, a prompt refund will be issued.
We require at least 48 hours’ notice before cancelling or rescheduling an appointment. This way, we have some time to try and fill the opening left in our schedule. We reserve the right to charge your account $50 if we are not notified at least 48 hours before your missed appointment.
We would like to take this opportunity to thank you for considering our practice for your dental care.
